You’re about to begin a new kind of intake experience, one designed to honor your time, your story, and the complexity of your health.

What is Wellroot?

The Wellroot AI health analysis system I personally developed works alongside my clinical expertise to study the details you share and extract the most meaningful patterns.
This allows me to design a plan that is more precise, more personalized, and immediately actionable.

To make this possible, the intake is completed in 4 short sections, each submitted separately. These 4 pages are protected and, upon appointment confirmation, you will receive the password to begin the intake.

How to Complete Your Intake

Please read carefully before beginning.

The pages are password-protected:

Your appointment confirmation email will provide the unique password to enter the encrypted form.

1. Your Name Must Appear on Every Section

Because each section is submitted individually, you must enter:

  • First Name

  • Last Name

At the top of every section.

This ensures all four submissions can be correctly merged into your complete chart.

2. Estimated Time to Complete

Here is the average time for each part:

  • Section 1: 6–8 minutes

  • Section 2: 8–10 minutes

  • Section 3: 6–8 minutes

  • Section 4: 5–7 minutes

Total estimated time: 25–30 minutes
You may complete them all at once or return later.

3. Be as detailed as you can

The more thoroughly you describe your history, symptoms, exposures, goals, and lived experiences, the more deeply I can analyze your case.

4. Save extra notes for the end

If you remember something later, Section 4 includes an open space for anything not covered in the earlier forms.

Once all four parts are received:

  • Wellroot AI organizes and analyzes your data

  • I review everything personally

  • A tailored health plan begins taking shape before your appointment

  • Our first session starts with clarity, not paperwork
